The Student Data Verification Forms are in the process of going home for your perusal and updating. Please take a few extra minutes to go over each section very carefully. If there is any information on the form that is no longer valid, please cross it out. Add any new information on the lines on the right side of the page. If you do not cross out the old information, it will stay in the database.
Please sign and return these forms directly to the office as soon as possible. It is very important for the safety of your child, that we have the most current, accurate information.
